Appointments
Children are enrolled in therapy following an evaluation in the Occupational Therapy Clinic. Placement is also possible for individuals who have had a recent evaluation or screening by a licensed occupational therapist, if adequate records are available. All applicants must have a completed case history form in order to be scheduled for either an evaluation or therapy.
