Parents, please read this before completing any application materials Before filling out application materials on the bottom of this page please contact Ken Cressman via email or at 404-233-5332 ext.3193. Please leave your name and phone number so you may be contacted to discuss the correct camp or service for your child. Failure to do so may result in an application not being accepted for the summer program. Applications for academic camps must also include the following additional pieces of information before being considered: Case History Questionnaire School Questionnaire – To be completed by child’s teacher Psychoeducational Evaluation (Not required for all camps or services) And/or any other testing completed by Speech Language Pathologists or other learning specialists Applications for Summer Excursions and Summer Learning Lab must include a Psychoeducational Evaluation and is highly recommended for the other academic camps. Please send all items together (with the exception of the School Questionnaire to be completed by your child’s teacher). 	Financial Aid
	Children applying to the Summer Program are considered without regard to financial circumstances, giving all applicants equal opportunity for acceptance. A financial aid fund is maintained to assist families who require financial assistance. After a child is accepted into the Summer Program, a financial aid application may be submitted.
